Disabled fans will be asked to pay the appropriate amount from the list above and, subject to receiving the relevant DLA or PIP payment at the mid or higher rate, will they also be entitled to a carer ticket free of charge.
Any supporters wishing to purchase a concession ticket will need to provide proof of age.
This game is not an all-ticket fixture at present for City fans although a 'pay on the day' option is subject to availability.
Tickets will go OFF SALE from the Bantams Ticket Office at 5:00pm on Thursday (28 October 2015). Any tickets purchased by supporters after 5:00pm on Tuesday (26 October 2015) will NOT be posted out.
They will only be made available for collection.
Any tickets not collected from the Bantams Ticket Office by 5:00pm this Thursday will be taken to Millwall's ground for collection on match-day.
The Bantams' previous visit to The Den saw over a thousand City supporters make the journey south to see Parkinson's men play out a thrilling 3-3 draw with the then-Championship side Millwall in the FA Cup third round last season.
